When you need this form

Handovers, acquisitions and disposals are where maintenance history goes missing, and the gap becomes your liability. The same applies to a new building coming onto your books, or a board that wants proof the plant is being looked after. Owners corporations are increasingly asked for long-range capital works planning per asset, and that planning is only as good as the maintenance record behind it.

Photos are the difference. A line in a spreadsheet saying "pump inspected" convinces nobody twelve months later. A dated image of the plant, the reading and the defect, pinned to the site and attached to the asset, settles arguments with owners, insurers and incoming managers before they start. Exceptions and escalations carry the same weight.

What good looks like

• Each activity tied to a named asset and its location.
• Before and after photos, dated and pinned to the site.
• Observations separated from exceptions, so real problems are not buried.
• Escalations recorded with who was notified and what they authorised.
• Authorised sign-off closing every entry, with time on site.
