When you need this form

Before a contractor starts high risk work on your asset, they submit the method statement covering it. Working at heights, confined spaces, live electrical, demolition, plant. This form collects the scope, the hazards, the controls and the crew consultation in one submission, so the person controlling the site has the document on file rather than a promise it exists somewhere.

Emailed SWMS pile up unread and nobody can prove which version was used. A submitted form timestamps the version, names the authoriser, and captures every worker who signed on to the method. If an incident is investigated, you show the controls that were in place and the people who were briefed, without reconstructing it from memory.

What good looks like

• Work activity and location defined tightly, not 'general maintenance works'.
• Hazards listed per task step, with controls ranked by hierarchy.
• Plant, equipment and licences required for the work named explicitly.
• Consultation recorded: who was briefed, when, and by whom.
• Contractor representative authorisation, with the version and date on the document.
