When you need this form
Before the first start of the shift, plant gets checked: elevated work platforms, scissor lifts, generators, saws, pressure units, vehicles. Principal contractors ask for pre-starts as part of the site pack, and refuse mobilisation without them. Australian WHS practice puts the duty on the person using the plant to confirm it is safe, and a defect found in the yard costs far less than one found at height.
After an incident, the first request is the pre-start for that machine on that day. A book in the ute cab is easy to backfill and easy to lose. A live check is stamped with the operator, the time and the location, and a photographed defect goes straight to the person who can authorise a repair.
What good looks like
- Plant identified by asset or registration number, not "the small lift".
- Every check answered pass or fail, with faults described in words.
- Photos of the defect: guarding, hoses, tyres, leaks, damaged controls.
- Corrective action recorded: tagged out, repaired on site, or replaced.
- Operator and supervisor sign-off before the plant is used.
Plant & equipment pre-start checklist, answered
Is this a PDF or Word template?
Neither. It is a live form: your crew opens it on their phones, ticks it through, attaches photos and signs on the glass. Every submission is timestamped, GPS-pinned and stored as proof.
When do I need a plant pre-start checklist?
Before plant or equipment is used each shift, and again after a change of operator or a repair. Principal contractors usually require completed pre-starts before mobilisation on site.
What happens if a pre-start check finds a defect?
The plant is taken out of service until the fault is assessed. Record the defect, attach a photo, note the corrective action taken and have a supervisor authorise the return to use.
Can I buy just this one form?
It comes with the plan: this form, the other 276 in the library and Eaco AI for $29 a month, GST included. Cancel anytime.
