When you need this form
Events generate medical work: heat, alcohol, crowd crush at the barrier, sprains on stairs, and the occasional serious presentation that needs an ambulance. Venue and event contracts usually require a medical log for the whole operating period, patron by patron. Recording each presentation as it happens keeps the event controller across load, and it is the record the venue, the promoter and the local authority all ask for afterwards.
Debriefs happen the next day, when nobody remembers exact times. Filled live, each entry holds its own timestamp and location, so a spike at one gate or a cluster near a stage is obvious on the report. Handover between shifts carries the open cases forward rather than losing them, and the promoter gets a clean summary instead of photographs of a notebook.
What good looks like
- Each presentation timed and located: gate, stand, bar or stage.
- Presenting complaint and treatment given, recorded in plain language.
- Outcome logged: returned to event, ejected, referred or transported.
- Ambulance or police involvement noted with the time of the call.
- Open cases carried into handover and signed by both supervisors.
First-aid & medical log, answered
Is this a PDF or Word template?
Neither. It is a live form: your crew opens it on their phones, ticks it through, attaches photos and signs on the glass. Every submission is timestamped, GPS-pinned and stored as proof.
What should an event first aid log include?
Every presentation with its time and location, the complaint, treatment given, the outcome, and whether an ambulance or police attended. Add handover notes so open cases pass cleanly between shifts.
Why do venues ask for a medical log after an event?
It shows how many patrons needed care, where the pressure points were, and how quickly they were treated. Venues, promoters and insurers use it to review crowd management and set staffing for the next event.
