When you need this form
New starters, contractors on site for a week, and crews replacing worn gear all need PPE recorded at the point of issue. Harnesses, respirators, hearing protection and hi-vis then need inspection on a cycle, because gear degrades. Australian WHS practice puts the duty on the employer to provide, maintain and check equipment, and a register is how that duty is shown rather than claimed.
