When you need this form

Gastro sweeps through a childcare room. A cluster closes a boarding house. A vomit incident happens in a school corridor at recess. Each triggers a response clean that differs from the daily scope: stronger products, restricted access, disposal rules and a return-to-use check. Education and care sites expect that response to be documented, because parents and their governing bodies ask.

Term-based audits arrive long after the outbreak. What holds up is a record made while the area was still closed: the products used, the contact time, the room signed back into service and by whom. Photos of the affected area and the barrier signage do more for a client's confidence than an emailed assurance ever will.

What good looks like

• Illness type, affected rooms and the date the response was triggered.
• Products and dilution used, with contact times observed per surface.
• Access restrictions and signage put in place, photographed on site.
• Waste and soiled linen handled and disposed of per site procedure.
• Room released back to use, with the time and authorising staff member.
