When you need this form

Centre floors are the asset and the risk. Polished stone in the mall, terrazzo at entries, sealed vinyl back of house, all walked by thousands of people a day. Burnishing, stripping, sealing and stone honing have to be planned around trade hours and wet floor exposure. Centre managers care because slip claims are expensive, and because a dull mall floor is the first thing a retailer complains about.

After an incident, the question is always when that area was last treated and what was applied. A live schedule answers with a date, a product, a crew and photos, including the signage that was out during the works. Paper schedules cannot show the barriers were up at 2am. That evidence protects both you and the centre.

What good looks like

• Floor types mapped by area: stone, terrazzo, vinyl, tile, back of house.
• Scheduled activity named: burnish, strip and seal, hone, polish, grout clean.
• Products and coat counts recorded against the surface they were used on.
• Night works windows, barricading and wet floor signage evidenced with photos.
• Approval from the centre before the works and sign-off after.
